Nurse Practitioner – Palliative Care Center, Wilmington - 1.0 FTE The Nurse Practitioner provides direct patient care for patients with life limiting illnesses. The patient population can include adults (45-64 years) through geriatric patients (65+ years). Is responsible for using expert skills in history taking, physical examination, initiation and interpretation of tests, analysis of data, and the implementation and evaluation of plans of care. Effectively manages the Plan of Care for each patient based on clinical knowledge, data, patient preference and benefits to the patient. Management of each patient also includes appropriately and effectively delegating patient care duties to other members of the team in order to assure optimal patient outcomes. Works with Palliative Care Center physicians in the hospital, nursing homes and physician’s office and patient’s homes. The Nurse Practitioner is responsible for continuing his/her own personal development to enhance practice and the profession of Nursing. Graduate of a master’s degree in nursing program with a major or post-master’s as a nurse practitioner required. Current license to practice professional nursing in the State of North Carolina required. Approval for practice as a nurse practitioner by the North Carolina Board of Nursing and the North Carolina Board of Medical Examiners required. Minimum of five years general medical experience in inpatient, outpatient or nursing home settings required. Minimum of two years in geriatric nursing and two years as a nurse practitioner required. Experience in an acute care setting desirable. Demonstrated competence in leadership and teaching. Demonstrated achievement and familiarity with administrative nursing techniques.
